Korlym contains Mifepristone, potent blocker of hormones progesterone and cortisol. Mifepristone blocks a natural hormone (progesterone) that is required for your pregnancy to continue. It is usually used in combination with another drug called misoprostol.

Also, Korlym is a glucocorticoid receptor inhibitor that is used in controlling high blood sugar associated with Cushing's syndrome. The potent metabolic effects of excess cortisol may affect many body systems, and patients may also suffer from many problems, including depression, cognitive difficulties, psychosis, diabetes, obesity and muscle degeneration.

Drug Interactions

The following drugs interact with Korlym and may increase or decrease its potency:

  • Cholesterol lowering drugs such as Lovastatin, Simvastatin
  • Antifungal drugs like Ketoconazole, Fluconazole
  • Antidepressants such as Fluoxetine, Citalopram
  • A blood thinner or anticoagulant such as Warfarin or Coumadin
  • Ergot containing drugs such as Ergotamine or Dihydroergotamine
  • Antiviral drugs to treat HIV/AIDS or hepatitis C like Zidovudine, Abacavir or Lamivudine
  • Steroid medicines such as Dexamethasone, Prednisone or Methylprednisolone
  • Drugs used in transplantation such as Sirolimus, Tacrolimus

Important Limitations for Use

  • The drug’s use is contraindicated in pregnant and breastfeeding women.
  • Korlym use is contraindicated in patients who need concomitant treatment with systemic corticosteroids to cure their medical disorders (for example, immunosuppression after organ transplantation) as the drug results in antagonism of the effect of glucocorticoids.
  • Patients taking lovastatin, simvastatin, and CYP3A drugs such as Dihydroergotamine, Fentanyl, Ergotamine, Sirolimus, Cyclosporine, Quinidine, and Tacrolimus, due to an increased risk of adverse reactions.
  • If you have or had a history of bleeding disorder or are taking drugs to thin your blood or anticoagulants, avoid taking this drug.
  • Its use is contraindicated in women with a history of unsolved vaginal bleeding issues and women with endometrial cancer.
  • Do not take this drug if you have prior hypersensitivity or allergic reactions to Mifepristone or to any of the drug ingredients.
  • Do not use this drug in the treatment of patients with type 2 diabetes except it is secondary to Cushing's syndrome.
  • Avoid taking Korlym if you have had an organ transplant.
  • You should not drink grapefruit juice while you take this medicine.

Prescribed Intake of Korlym

Each Korlym tablet contains 300mg of Mifepristone. Korlym is generally taken orally once a day with a meal. Swallow this tablet whole without splitting, crushing or chewing them. Your physician may change your dose if needed.

What will happen if you forget to take a dose?

As soon as you remember, take the missed dose. If it is the time to take the next dose as per your schedule, do not take an extra dose. Do not take double dose of the medicine to make up the missed dose.

Possible Side Effects

Serious side effects of this drug may include:

  • abnormal vaginal bleeding
  • new or worsening medical complications
  • unusual weakness, nausea, tired feeling
  • low blood sugar, symptoms may include weakness, sweating, headache, hunger, confusion, fast heart rate, irritability, dizziness, or feeling nervous
  • a light-headed sensation, like you may pass out
  • low potassium levels, which may result in extreme thirst, confusion, uneven heart rate, leg discomfort, increased urination, drooping feeling or muscle weakness
